Springhill Hospice at Home
The new Hospice at Home service is off!
Following its launch day on Friday 1 June, Hospice at Home and Hospice at Home plus are now officially open for business. The extension to the services already offered at Springhill Hospice received a warm welcome from the packed room who watched the presentation by Sheila Johnson, Director of Clinical Services at Springhill Hospice. Sheila Johnson explained that the service is to work in conjunction with other service providers offering assistance to end of life patients, such as the District Nursing Team, Home Care Providers and Macmillan Nurses.
“The services we offer at Springhill Hospice are absolutely fantastic, but quite often patients say that they wish that they could have the services that we offer at Springhill but in their own home. This exciting new service development will enable more patients, and their families, to have a choice about their care at the end of their life.”
In addition to the 24 hour Hospice at Home service, Springhill Hospice at Home Plus, in partnership with RMBC Adult Services, provides a Night Sitting service for patients at the end of life, where additional support during the day is not necessary.
Patients can get access to the services if they meet the following criteria:
- The patient is 18 years of age or over.
- The patient is in the last days of life.
- The patient and family have indicated that home is the patient’s preferred place of care.
Referrals can come from their current Community Healthcare Professional ie GP, District Nurse, Macmillan Nurse, Social Worker.
